Female to Male - Chest Masculinization

Subcutaneous mastectomy through a periareolar incision is the simplest and most direct method of reducing breast volume to create a male looking chest. It is performed under general anesthesia, through a semicircular incision made under the areola at the junction of normal skin and areola skin. The incision leaves minimal scarring. This operation is ideal for moderate sized breasts where the nipple and areola are at least 3cm to 4cm above the inframammary fold. A compression dressing is useful to reduce swelling during the initial post operative period. Hemovac drains are usually recommended for the first 48 hours.

For females with moderate to large breasts, where the nipple areola complex is below the infra-mammary fold, the reduction surgery is planned to elevate the areola and reduce and sculpt the remaining breast tissue to look like a male chest. This can be done by excising breast tissue through a transverse incision in the inframammary fold while preserving the blood supply to the nipple areola complex. The areola is elevated on a vascular pedicle without being detached and is sutured in its new position through a predetermined matching opening approximately 4 cm above the IMF which is the normal position for the areola in a male chest. Compression dressings in this type of reduction are not recommended during the first post postoperative week to minimize constriction of the areolae blood supply. A small hemovac drain is usually left in place for 48 hours.
 

Male to Female - Breast Augmentation

In the surgical sense, a male breast augmentation is similar to a female breast augmentation.  The breast implants can be either saline or silicone or placed under or over muscle. The risk of side effects for any breast augmentation is not gender related.
